Who knows as to connect it to the HTML form??? not to NewForm.aspx and to HTML form

Have you searched the forums here in SPServices for related topics? This question comes up allot and is not something the utility was designed to support. So you may have to roll your own tool/code to make it work. That being said, here is one guy I think got it to work a few weeks ago:
