SPLookupAddNew Issue in SP2010

Marc, this worked like a charm in an SP 2007 site. I'm now trying this on a list in SP 2010. I have modal dialogs on this list (ContractList) turned off, but am not getting the desired results. Here's my code -- any ideas on what I'm missing? The display name of the lookup column is Vendor. Although I have debug turned on, I'm not seeing any error messages. I have inserted the code in the source editor of an HTML Form web part on the page. I know the javascript is running, because when I insert an alert in the code it fires when the page loads.

I don't see anything obviously wrong here. What's happening in the form? Are you getting the link, or is nothing showing?
