SPArrangeChoices With Custom Form Not Working

I think I need a little assistance with the SPArrangeChoices!

I have created a custom form for one of our 2007 SharePoint sites. The following code is added to the page:

    alert('document.ready fired');
    $().SPServices.SPArrangeChoices({
      columnName: "My immediate supervisor let me know when I was doing a good job",
      perRow: 4
    });
    alert('SPServices.SPArrangeChoices completed');
  });
</script>

 I inserted the alerts to verify that the js was being referenced properly - both of them pop up with no problems.

And I have the following for the field:

But when I load the page, the radio buttons are still vertical. The section works fine on a default form, but in my custom form it doesn't work at all. Help :(

Try replacing your long columnName in the code with the actual field internal name which I believe you already have [My_x0020_immediate_x0020_supervi].

Thanks for the response Dan. I replaced the code name in both spots with the internal field name and it still is not showing up properly.

If the custom form isn't in the list context (either /Lists/listname or /listname for a Document Library) then the function won't work. You'll need to provide the listname in the options.
